If you are a new patient to Health Plus General Practice and you are visiting us for the first time you can now fill in your ‘new patient registration form’ and ‘new patient history questionnaire’ here, saving you time before your scheduled appointment.

Simply complete your forms and either email it to us at contact@healthplusgp.com.au or bring them with you to your appointment.
